Delaware Receives Approval from CMS for Health Home Services

Delaware Division of Developmental Disabilities Services (DDDS) received approval from CMS last month for SPA #18-006 “ACIST Health Home Services”. The SPA was submitted to expand the Assertive Community Integration Support Team (ACIST) program, which supports individuals with Severe and Persistent Mental Illness (SPMI) and intellectual and developmental disabilities (I/DD).

DDDS will be eligible to receive a 90% enhanced Federal Medical Assistance Percentage (FMAP) from October 1, 2018 through September 30, 2020 for the specific health home services in Section 2703 of the Affordable Care Act.

DDDS is required to collect health home quality measures data from the health home providers.  These quality and utilization measures will be used for ongoing monitoring and evaluation purposes across all state health home programs.
